Tracy Anderson-Haag is a Clinical Pharmacy Specialist in Kidney Transplantation at Hennepin Healthcare and is an Adjunct Associate Professor at the University of Minnesota College of Pharmacy. She earned her Doctor of Pharmacy degree from the University of Minnesota and completed a Nephrology/Transplantation Residency at Hennepin County Medical Center. She is board certified in pharmacotherapy and solid organ transplant. She has practiced for over 25 years in transplant and nephrology pharmacy. Tracy is responsible for medication management of inpatient and ambulatory transplant recipients, living kidney donors and CKD patients. She is also involved in policy development and clinical practice expansion. She serves as the Engagement Co-Lead for the Advancing Kidney Health through Optimal Medication Management (AKHOMM) initiative, which aims to ensure every person with chronic kidney disease receives optimal medication management through team-based care including a pharmacist.
